Six health insurance plans to be offered to uninsured workers

Level One: Discount Network. Not insurance, this level allows workers to buy into the discounts negotiated by health insurers. Cost: $4.41 per month.

Level Two: Scheduled Wellness Benefit. It covers 80 percent of office visit costs and 100 percent of preventive care, up to plan maximums. Also pays $20 per prescription for five prescriptions per year, along with two dental visits and one vision visit per year. No hospitalization coverage. Cost: About $50 per month.

Level Three: Scheduled Outpatient Benefit. Similar to Level two, but includes 100 percent of office visits and adds outpatient surgery. Cost: Ranges from $88 to $140 per month.

Level Four: Scheduled Inpatient and Outpatient Benefits. Similar to level three, but also pays $800 per day for inpatient stays up to 30-day maximum, and covers 100 percent of emergency room visits. Cost: Cited examples ranged from $100 to $167 per month.

Level Five: Major Medical with health savings account. Deductible of $2,000 for single coverage or $4,000 for family coverage. Covers 70 percent of costs, with the patient paying the balance. Also includes prescription drug coverage. Cost: Cited examples ranged from $71 to $336 a month.

Level Six: Limited Major Medical. Same as level five, with deductibles of $1,100 for a single person and $2,200 for a family. Cost: Cited examples ranged from $71 to $395 per month.
